The Transitions Program uses team-delivered interventions designed to "bridge the gap" and provide therapy, linkage and practical support to empower young people and help them gain independent life skills. By surrounding the young adult with an individualized and flexible team with a mobile nature this results in interventions occurring within the environment in which the problems are experienced. 

 
For many young people, especially those who have been in receipt of supportive Human Service Interventions, the "launching" process is challenging and overwhelming. Many do not make a successful transition to adulthood.
The target population for this program is characterized by higher rates of school dropout, homelessness, unemployment, mental health issues,  substance abuse issues and repeated hospitalizations and incarceration.

Becky Lepley, BA

Becky Lepley, BA is a Transitions Specialist Supervisor in the Transitions Program.

She graduated from West Virginia University in 2016 with a major in Psychology. Becky began her career as a drug and alcohol counselor before spending five more years in the drug and alcohol field in various positions. She has also worked with the intellectually disabled community. Becky began her career at Regional Counseling Center in the Child and Adolescent Partial Program before transitioning to the Hoarding Program and then the Transitions Program in January 2024. Becky is also a co-facilitator of the LGBTQ Group at RCC. She enjoys working with children, the LGBTQ community, and the ID population. Becky is currently furthering her education and is pursuing her Master's Degree in Clinical Mental Health Counseling from Pennwest University. In her free time, Becky can be found curled up with a good book, spending time with her family and cat, or exploring the outdoors! 


Sarah Young, MHP. MSW

Sarah Young was hired in February 2025 as a mental health therapist in the Transitions Program at RCC. She graduated from the University of Pittsburgh with her bachelor's degree in social sciences with a focus in Psychology, Sociology and Anthropology. She then went on to serve as a Resident Director at both the Pitt-Bradford and Pitt-Greensburg campuses. Sarah earned an MSW degree from Pitt-Oakland’s School of Social Work in 2018 and worked for five and a half years as a Child and Family Therapist with the Family Foundations program at FSCAS. She obtained an MHP after completing the 3-year ESFT training program from the Philadelphia Child and Family Training Center. Currently, Sarah is working to obtain an LCSW. She loves collaborating as part of a team and supporting co-discovery that all people are capable of success and worthy of care!

Sarah enjoys a vast catalog of music, watching Penguins hockey, cheesy Dad jokes, human rights activism, going to the beach and her entirely one-sided, long distance dream romance with actor Henry Cavill. 

Jacy Blum, MPH

Jacy Blum, MPH, is a Mental Health Therapist in the Transitions Program. He joined the Transitions team at RCC in March of 2024. Jacy graduated from Slippery Rock University in 2019 with a Bachelor's degree in Exercise Science. He then returned to Slippery Rock University to obtain his Master’s degree in Public Health. Prior to joining the Transitions team at RCC, he worked as a Direct Support Professional, supporting individuals with intellectual disabilities. Since joining Transitions, he has become a co-facilitator for a weekly DBT Group, as well as facilitator for several other groups amongst Transitions. He enjoys working cohesively as a team to best serve Transitions consumers and support them as they actively attempt to make change in their lives. He regularly attends events hosted by FLEX- Venango County’s Young Professional Organization and the Venango County Chamber of Commerce to network and spread awareness of all that the Regional Counseling Center has to offer. In his free time, Jacy can usually be found enjoying the outdoors with his partner, being a couch potato with his dog, Reuben. He is an avid Pittsburgh sports fan who loves attending sporting events with his friends and family.

Typical program participation is 12 months. 
This program is 100% voluntary.

Individuals will likely have: 

  • Must be 18-25 years of age
  • Have VBH insurance
  • Demonstrate impaired functioning in home, school, or community
  • Dysfunctional family connections, or no family support
  • Transitioned from foster care or other structured living arrangements.
  • Been involved with at least 2 human service agencies
  • Agree to participate in the program voluntarily
