The Transitions Program uses team-delivered interventions designed to "bridge the gap" and provide therapy, linkage and practical support to empower young people and help them gain independent life skills. By surrounding the young adult with an individualized and flexible team with a mobile nature this results in interventions occurring within the environment in which the problems are experienced. 

 
For many young people, especially those who have been in receipt of supportive Human Service Interventions, the "launching" process is challenging and overwhelming. Many do not make a successful transition to adulthood.
The target population for this program is characterized by higher rates of school dropout, homelessness, unemployment, mental health issues,  substance abuse issues and repeated hospitalizations and incarceration.

Lindsay Carter Assistant Director LPC

Lindsay Carter, LPC Assistant Director (Program Director/ Mental Health Clinician/Clinical Manager): I obtained my Bachelor of Arts degree in Psychology, minoring in child development and family relations, sociology, with a concentration in sexual education and gender studies. I then went on to complete my master’s degree in Clinical Mental Health Counseling in 2020 and obtained my LPC in 2022. I joined the Intensive Transitions Team in May 2009 as the first Transitional Specialist. In September 2018 my position transitioned into mental health clinician as well as program director. We are a team-delivered intervention which is one of the best parts of my job. I really enjoy working with my teammates and finding support and growth within each of them. We are a passionate group of professionals that do intense work with transition-aged youth to help create lasting change in their lives. I have trained under the ESFT model since 2009, and was recently certified in DBT. I also sit on the Venango County Social Determinants of Health board.

Nicole Knox MS

Nicole earned her bachelor's degree in rehab science with a concentration in developmental disabilities. She continued her education obtaining a masters degree in clinical mental health counseling and recently received her LPC licensure. She has been practicing in the mental health field for 12 years and has a wide variety of experience. She’s been with RCC going on four years gaining experience during her internship in the IOP program. After graduation with her masters, she was hired as a clinician for the school based program, then moved to the outpatient program and currently in the transitions program where she has found her niche. She’s very passionate about advocating for her clients and their families. She has specialized training in dialectical behavior therapy, geriatric mental health, PTSD, and continues to receive training and education as a part of her continuing education requirements for her degree. When she is not a therapist she is a mom of some amazing kiddo’s and three dogs. She loves digging in the dirt and creating beautiful landscapes while listening to rock and roll or alternative music. She is also a collector of house plants and loves to encourage others to find the joy in caring for beautiful plants as she feels it is “very therapeutic”.

HaleyBell BS

Haley Bell BS, (Transitional Specialist): Haley joined Transitions in March 2018. She completed her undergrad in Public Health from Purdue University. She is working towards her Master's degree in Clinical Mental Health Counseling. Haley has worked as a Pregnancy Counselor at Matrix Life Care Center in Lafayette, IN. She then moved on to the Department of Child Services in Indianapolis as a Family Case Manager. Haley enjoys working with young adults, specifically ones who are pregnant and/or parenting. She also has a special interest in working with clients diagnosed with Borderline Personality Disorder. Family Systems Therapy is a therapeutic focus she hopes to develop during her graduate studies. Outside of work, I enjoy the outdoors and spending time with my family. 

Becky Lepley, BA

Becky Lepley, BA is a Transitions Specialist/ Transitions Housing Case Manager in the Transitions Program.

She graduated from West Virginia University in 2016 with a major in Psychology. She began her career as a drug and alcohol counselor before spending five more years in the drug and alcohol field in various positions. She has also worked with the intellectually disabled community. She began her career at Regional Counseling Center in the Child and Adolescent Partial Program before transitioning to the Hoarding Program and then the Transitions Program. She enjoys working with children and the ID population. In her free time, she can be found curled up with a good book, spending time with her family and cat, or exploring the outdoors!

Katie McMichael M.S. LPC Transitions Program Director

Katie joined the Transitions Team as a Mental Health Clinician in October 2021. She received her Masters in Mental Health Counseling from Clarion University. Her experience prior includes providing Family Based and Drug and Alcohol Therapy to a variety of populations and a variety of settings. Katie is passionate about helping others and enjoys working with adolescents and young adults but enjoys working with all populations. Originally hailing from Tionesta PA , Katie enjoys being around animals and the outdoors. When Katie is not working mostly found spending time with her husband and 6 children taking care of their zoor of animals, playing board or video games, spending time outside, or going on road trips.

We provide a 24-hour crisis line.
Services will be team delivered in the home and community. 
Outcomes will be measurable. 
Typical program participation is 12 months. 
This program is 100% voluntary.

Individuals will likely have: 

  • Must be 18-25 years of age
  • Have VBH insurance
  • Demonstrate impaired functioning in home, school, or community
  • Dysfunctional family connections, or no family support
  • Transitioned from foster care or other structured living arrangements.
  • Been involved with at least 2 human service agencies
  • Agree to participate in the program voluntarily
